Technical Assistance:
Consultation provided on the fall zone for the volcano climber , which requires 9 inches of resilient surfacing ( mulch).

Once the mulch is compacted, its no longer considered a resilient surfacing so you will need to replenish or fluff the mulch to ensure its resiliency. Keep in mind that if the mulch has degraded and starts to break down to dirt, the only remedy is to replace the mulch.

Portable slides and any play equipment a child can climb on and/or fall from requires at least 6 inches of resilient surfacing and possibly more depending on the height of the structure and the type of resilient surfacing, please refer to Figure 2: Compressed Loose Fill Synthetic Materials Depth Chart .

Make sure the attendance clipboard is present in the classroom with the children.

Either the door to the kitchen must remain locked or you can keep all hazardous substances in a locked cabinet in the kitchen.

Consultation provided on standard 240 C regarding components of orientation training.

Make sure all staff are documenting the time parents were told of injuries to their child.

Comments:
An unannounced renewal inspection was conducted today with the director, who was present with 18 preschoolers with 4 staff providing supervision. The inspection took place from 11:55 am to 2:35 pm. The ages of the children present ranged from10 months to 4 years. Most of the children were finishing lunch, which consisted of salisbury steak, mash potatoes, mixed fruit and milk.Two vans were inspected as well as the playground.
The fire inspection was dated 9/14/21 and the health inspection was dated 2/11/22.
The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on a review of six children's files and interview with administrative staff, it was determined that one child did not have a physical on file within 30 days of enrollment.
Evidence:
Child A enrolled on 9/6/21 and does not have a physical on file.

Plan of Correction: We will obtain the physical from the parent.

Standard #: 8VAC20-780-160-A
Description: Based on a review of 5 staff files and interview with administrative staff, it was determined that one staff did not have a tuberculosis test/screening on file at the time of employment and before working with children.
Evidence:
Staff A was hired on 8/26/22 and does not have a tuberculosis test/screening on file.

Plan of Correction: We will obtain the tuberculosis test/screening.

Description: Based on a review of five staff files and an interview with administrative staff, it was determined that one staff's orientation training did not have all of the required topics.
Evidence:
Staff A was hired on 8/22/22 and the orientation training did not include prevention of sudden infant death syndrome, safe sleep practices, prevention of shaken baby syndrome, abusive head trauma, dealing with crying babies and distraught children. prevention and response to emergencies due to food and other allergies, prevention and control of diseases.

Plan of Correction: We will audit all staff files and provide the additional training to all that needs it.

Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that the center was not maintained in a safe condition.
Evidence:
1. The Licensing Inspector observed screws protruding more than two threads on the playground fence and gate.
2. The Licensing Inspector observed an electric fan sitting on a shelf, with the cord plugged into the outlet. The fan cord was in reach of the children and if pulled, the fan would fallen off the shelf and created a potential safety issue.

Plan of Correction: We will remove the fan to ensure it doesn't present a hazard to children and we will cut and file the bolts/nails.

Standard #: 8VAC20-780-330-B
Description: Based on observation, it was determined that there was insufficient surfacing under and around the volcano rock climber on the older playground.
Evidence:
The rock climber has a fall height of 8'2", therefore requires 9 inches of resilient surfacing underneath and six feet around the climber. The resilient surface was compact and therefore was not a resilient surface surrounding the climber.

Plan of Correction: We will place the volcano climber off limits until we can add at least 9 inches of mulch in all of the fall zones.

Standard #: 8VAC20-780-500-B
Description: Based on observation, it was determined that the diapering surface was not used only for diapering and cleaning children.
Evidence:
The Licensing Inspector observed two pairs of infant shoes stored on the diaper pad. Items stored on the diapering pad risks cross contamination.

Plan of Correction: The shoes were removed from the diapering area.
