Description: Based on record review the center did not obtain the results of a criminal record or central registry check from a state where the employee has resided within the preceding five years.
Staff 2's record did not contain the results of a criminal record or central registry check from a state where the staff member resided within the previous five years.

Plan of Correction: Astar Explorer submitted the background check to Richmond central registry office soon after she sworn before hiring[sic]. I talked to of central registry office at least 4 times about the case. Please see attached emails to the office. It is overwhelming volume of background checks that delay [] office work.

Description: Based on record review, one staff member did not complete a sworn statement or affirmation prior to the first date of employment.
The sworn statement for Staff 1 was completed 20 days after hire.

Plan of Correction: [Staff 1] completed her sworn disclosure prior to the first date of employment. But her manager accidentally mislocated [sic] this file and asked [Staff 1] to do it again That is why the second sworn statement is 20 days later.

Description: Based on record review, not all staff member's records contained all required information.
The record for Staff 2 did not contain documentation that two or more references were conducted.

Plan of Correction: It is Astar Explorer's hiring procedure to have 2 references checked before a job offer letter sent out [sic]. However we did not keep those verbal reference check over the phone record keep emails or recommendation letters on file. From now on, we will keep these phone reference records. For the current teachers or staff's file, we will redo the reference check and keep these references in each staff's file.

Standard #: 8VAC20-780-550-D
Description: Based on documentation review, there was no evidence that a monthly practice evacuation drill was practiced.
There was no documentation available on the date of inspection that recorded dates and times of monthly evacuation drills.

Plan of Correction: We had a meeting for this issue and listened to everyone's option on this evacuation drills,. The final decision is that we will do a monthly evacuation drill in the third week Wednesday of each month.
