Comments:
An unannounced Renewal Inspection was conducted today. A total of 50 children with 9 staff were present. Some classes were observed outside washing hands and eating snack outside. Children were observed transitioning to inside or to the outside amphitheater for planned activities. Observation of all classrooms, bathrooms, playground and some of the nature trails was made. Activities today included the color "blue". Artwork was either on display or drying that focused on the color blue. Blue crepe paper, feathers and glitter were used for some of the projects. All four classes rotated between centers located in the classrooms, hallway and another activity room. Staff were strategically positioned to ensure adequate supervision of all children. Circle time that included reviewing the number of days in the month and identifying green and red apple patterns kept the children actively engaged. All new staff records were reviewed (3) and a follow up review of 1 additional staff record was reviewed. A total of 5 children's records were reviewed. This inspection occurred between 10am and 1pm. Thank you for your cooperation today. Please email me with any questions at: lisa.hudson@doe.virginia.gov

2. Staff #3 was hired on 08/10/2022.
3. A criminal record check request has not been made.

Plan of Correction: We will request this criminal record check from the state the staff resided immediately. We will send the results to the Licensing Inspector for review within 10 working days.
Ongoing, any staff who has resided out of state will not be hired until the results are received.

Standard #: 8VAC20-780-40-M
Description: Based on observation of the center, a current written list of children's allergies was not maintained in a confidential manner.
Evidence:
The list of children with allergies in the Dragonflies class was posted on the outside of a cabinet.

Plan of Correction: We will relocate all allergy lists for children inside the cabinets in each classroom.

Standard #: 8VAC20-780-240-I
Description: Based on staff record review, documentation of orientation training for one staff was not part of the staff record.
Evidence:
Staff # 4 was hired on 09/16/2022. Documentation of orientation training was not maintained.

Plan of Correction: The orientation training was completed. The staff has the training documentation and was not at school today. Will obtain the documented training and file in their record.

Standard #: 8VAC20-780-500-B
Description: Based on observation of the bathroom used for diapering, the receptacle used to dispose soiled diapers inside was not one that staff member's hand does not touch an exterior surface of the storage system during disposal.
Evidence:
The step on trashcan located by the diapering surface was not operating and required staff to touch the lid of the trashcan to open it.

Plan of Correction: We will replace the receptacle used for diaper disposal with one that does not require staff to use their hands.
