Comments:
An unannounced monitoring inspection was conducted on 09/19/2022. There were 25 children, ages 5 months-4 years, with 7 staff members present during the inspection. The inspector observed the following: free choice activities, diaper change, bottle feeding, lunch and outside time. A total of 3 children?s and 3 staff records, were reviewed. The inspector arrived at 9:00 AM and departed at 11:00 AM.

Violations:
Standard #: 8VAC20-780-280-D
Description: Based on observation in the kitchen, the center failed to ensure that all food was not stored with chemicals.

Evidence:
1. Observed a bleach bottle was sitting on the counter where food was being prepped.

Plan of Correction: The chemicals were removed during the inspection and will be monitored.

Standard #: 8VAC20-780-280-G
Description: Based on observation, the center to ensure that all substitute containers clearly indicated their contents.

Evidence:
1. The inspector observed a substitute container being used that was labeled as a disinfectant and the contents were not labeled as to what was actually in the container. The container contained bleach water.

Plan of Correction: When the chemicals are mixed the person will make sure that the correct label is on the bottle.

Standard #: 8VAC20-780-500-B
Description: Based on observation of the disposal system for diapering, the center failed to ensure that it was used in such a way that neither a staff member?s hand nor the soiled diaper touches an exterior surface of the storage system.

Evidence:
1. The disposal system had a lid that prevented the staff person to be able to not touch the exterior surface of the storage system.

Plan of Correction: The lids will be removed ASAP.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center to ensure that two bottles with infant formula was labeled and dated.

Evidence:
1. Two bottles sitting on the counter in the infant room was not dated or labeled with the owner?s name.

Plan of Correction: The bottles were labeled during the inspection.
