Comments:
An unannounced monitoring inspection was conducted on 9/22/22 from 3:45pm to 4:25pm with program staff. There were eight children in care, supervised by two staff. The children were observed doing homework or reading books and then cleaning up and washing hands for snack. Three child records and two staff records were reviewed. The Center had one staff with current certification in CPR and First Aid, and two staff trained in Daily Health Observation. One medication and its authorization form were reviewed and the center has one staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. Snack served was cucumbers, wheat crackers, and dip. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/30/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of two staff records, the center did not obtain documentation of the results of a search of the central registry within 30 days of employment for one staff member. Evidence: The record for Staff B, with a start date of 8/22/22, did not contain the results of the search of the central registry.

Plan of Correction: Call to get an update on her central registry.

Standard #: 8VAC20-780-70
Description: Based on review of two staff records, the center did not ensure that staff records contained all required documentation. Evidence: The record for Staff A indicated that the staff member was promoted to a program leader position on 8/10/22. There was no documentation submitted for Staff A that demonstrated that the individual possessed the education or certification required by the job position.

Plan of Correction: Manager will ensure that Staff A has the appropriate training.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances, such as cleaning materials, were kept in a locked place using a safe locking method that prevented access by children. Evidence: On 9/22/22 at approximately 3:50pm the Licensing Inspectors observed three bottles of disinfectant sprays on the bottom shelf of an unlocked cabinet in the classroom where children were present.

Plan of Correction: Livingston now has a locked box that all cleaners are kept in.

Standard #: 8VAC20-780-530-C
Description: Based on review of two staff records, the center did not ensure that there were at least two staff members certified in CPR present on the premises during the center's hours of operation and wherever children were in care. Evidence: On 9/22/22 there were two staff observed at the center. Record review revealed that Staff B has not yet completed CPR certification.

Plan of Correction: Staff will be CPR trained as soon as possible.
