Technical Assistance:
Technical assistance was offered related to: background checks, staff trainings, staff records, children?s records, MAT and daily health observation training for staff. Links to the department?s website and updated licensing standards were emailed to the center.

Comments:
An unannounced monitoring inspection was conducted from 11:00 am to 3:00pm. There were 126 children in care of 29 staff. Classroom and playground observations were made. Children were observed getting picked up for the day using the center?s car pool system. Children were also observed engaged in outdoor play and circle time. Handwashing was observed upon returning from the playground. Medications for children?s diagnosed allergies were on site with complete authorizations. 4 staff records, 4 children?s records, the center?s emergency plan and center policies were reviewed. Standard #: 22.1-289.035-A
Description: Based on records review, the center did not obtain results of a repeat 5 year central registry search for an employee for Virginia.
Evidence:
1. Staff # 1?s most recent central registry search was dated 2/6/17, and expired on 2/6/22.
2. Staff # 2 stated that a new search request was mailed on 9/22/22.

Plan of Correction: Business manager to follow up with applicable states. Communication is ongoing to correct .

Standard #: 22.1-289.035-B-4
Description: Based on records review and staff interview, the center did not obtain results of an out of state central registry search and sex offender search for an employee that resided outside of Virginia within the last five years.
Evidence:
1. Staff # 1?s first day of employment was 8/27/18. Staff # 1?s record indicated that she had resided in Pennsylvania within the last 5 years from the first day of employment.
2. Staff # 1?s record had a copy of the search request that was mailed. Staff # 2 stated that it was sent when Staff # 1 started her employment.
3. The results have not obtained to this date. A follow up was not completed to check on the status of the results.
4. A search of the Pennsylvania sex offender record for Staff # 1 was not completed before the first day of employment.

Plan of Correction: Business manager to follow up with applicable states to obtain current criminal background checks. Communication is ongoing to correct this situation.

Standard #: 8VAC20-780-160-A
Description: Based on records review, the center did not obtain tuberculosis (TB) screenings for employees that were documented within the last 30 days of the first date of employment.
Evidence:
1. Staff # 3?s first day of employment was 8/29/22. Staff # 3?s TB screening was dated 4/30/22.
2. Staff # 4?s first day of employment was 8/29/22. Staff # 4?s TB screening was dated 5/24/22.

Plan of Correction: Moving forward we will adhere to the new regulations as stated. Immediately when an employee is hired.

Standard #: 8VAC20-780-80-A
Description: Based on records review, the center did not maintain a written daily record of attendance that documented the arrival and departure of children in care, as it occurred.
Evidence:
1. Attendance records for September and October 2022 were reviewed.
2. Only one check mark was being documented against the name of each child in care, and did not show that the arrival and departure of children was being documented as it occurred.

Plan of Correction: Attendance forms have been amended to provide sign in and sign out info. Teachers have been informed of the change in procedure.

Standard #: 8VAC20-780-240-C
Description: Based on records review, staff orientation for new employees did not include all the specific topics as required by the licensing standards.
Evidence:
1. Staff # 3?s first day of employment was 8/29/22.
2. Staff # 3?s orientation did not include training on abusive head trauma.

Plan of Correction: Moving forward this training will be included in summer orientation. On 10/12/22 our all faculty meeting will be on abusive head trauma.

Standard #: 8VAC20-780-240-E
Description: Based on records review, staff did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment.
Evidence: Staff # 3?s first day of employment was 8/29/22. Staff # 3 did not have orientation training in first aid and CPR to this date.

Plan of Correction: Moving forward this will take place during summer orientation. CPR first aid scheduled 11/11/22 for all staff.

Standard #: 8VAC20-780-550-B
Description: Based on records review, the center?s emergency preparedness plan (EPP) did not contain procedural components for lockdown procedures.
Evidence:
1. The center?s EPP included procedures for shelter in place and evacuation drills, but not for lockdown drills.
2. The center?s EPP did not include procedures to reunite children with their parents or authorized persons.

Plan of Correction: Attached immediately. Post sign on main building/cottage.
