A complaint inspection was initiated by inspector on June 30, 2022 regarding concerns of staffing and
supervision. Interviews with staff and collaterals were completed, observations were made in the
center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
