Technical assistance was offered related to: Dismissal, attendance and activities for school age children; staff records (dates of hire and separation), attendance and supervision.
- Comments:

An unannounced inspection was conducted on 9/8/22 from 3:00pm to 5:30pm. The inspection was conducted in response to a compliant received by the department regarding ratios at nap time. Classroom observations were made at nap time, 4 staff members were interviewed, 4 staff records and attendance records were reviewed. Based on the preponderance of evidence, the allegations were not valid. Areas of non-compliance not related to the complaint were cited, and are listed in the violation notice. Standard #: 8VAC20-770-60-C-2 Complaint related: No Description: Based on records review and staff interview, the center did not obtain results of a Virginia central registry finding within 30 days of employment.
Evidence:
1. Staff # 4's first day of employment was 7/20/22.
2. Staff # 3 stated that she thought Staff # 5 had submitted Staff # 4's central registry search request, but it was never submitted.Plan of Correction: Staff # 5 was in charge of Staff records. Staff # 5 resigned without completing staff # 4's employment records. Before Staff # 4 returns to work the VA central registry finding will be in her file.
Standard #: 8VAC20-780-80-A Complaint related: No Description: Based on records review and staff interview, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. On 9/8/22, there were 14 children in the Sweet Tarts (4's) classroom with Staff # 1.
2. Staff # 1 stated that between 1pm -1:30pm Child # 1 arrived from his school. Child # 1's arrival was not documented on the attendance roster.
3. Staff # 1 stated that when Staff # 2 left for the day at approximately 2:15 pm, she took Child # 2 (Staff # 2's daughter) home with her. Child # # 2's departure was not recorded on the attendance roster.Plan of Correction: All children will be signed in and out upon arrival and departure from here forward.
Standard #: 8VAC20-780-210-A Complaint related: No Description: Based on records review, program leaders did not meet the qualification requirements for that position.
Evidence:
1. Staff # 3 stated that Staff # 4 was the program lead for the 2's classroom.
2. Staff # 4's record did not have any educational or professional ocumentation to show that she met the required qualifications to be a program lead.Plan of Correction: Staff # 5 was supposed to obtain staff # 4'squalification requirements. She did not complete the file prior to her resignation. Staff # 4 will not be a designated lead without her bachelor's degree in her file.
