Comments:
Thank you for your assistance during this unannounced complaint inspection. This on-site visit was conducted on September 22, 2022 from 9 a..m. through 10:30 a.m in conjunction with two other government entities. This inspection was initiated as a result of a complaint received by the Office of Child Care Health and Safety September 15, 2022. Observation and staff interviews were conducted relating to the allegation of special care provisions and emergencies on 9/29/2022 and 10/6/2022. The information gathered during this investigation does not support the allegation. Through the course of the investigation, other exceptions to compliance were identified and are noted on the violation notice

Complaint related: No
Description: Based on interviews, program did not ensure requirements were met when diaper ointment is used.

Evidence:
1. During interview on 10/6/2022, Staff #1 stated that Staff #2, mother of Child #1, determined that Child #1 needed diaper cream applied. Staff #1 explained to Staff #2 that the child did not have cream at the center and that another child's could not be used. Staff #2 retrieved the diaper cream of Child #2 and applied to Child #1.
2. There was no written parent authorization on file for Child #1.
3. Staff #2 did not record child's name, date of use, frequency of application, or any adverse reactions.

Plan of Correction: Staff #2 was released from employment on 9/15/2022. She was not given permission to use Child#2 cream on her own child, Child#1. Staff #1 clearly stated to Staff #2 that she could not use the cream; however, Staff #2 retrieved it anyway. Administrators reiterated the fact that at no point should cream be applied without permission and providing personal cream.
