Conversation was held in regards to medication authorization forms:
*Information in these forms must be clear and complete.
*An authorization form must include both the parent's and the physician's signatures for it to be considered complete.

A monitoring inspection was conducted on 10/12/2022 between the hours of approximately 7:15am and 8:25 am. There were 8 children present, ranging in ages from 6 years to 11 years, with 3 staff supervising. Children were observed arriving at the program and while playing cards and other games. Staff was observed greeting the children and engaging in conversation with them.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed.
