An unannounced renewal inspection was conducted on 10/5/22 from 8:45am to 11:25am with the Director and Assistant Director. There were 52 children in care, ranging in age from six-months to five-years-old, supervised by 10 staff. The children were observed participating in circle time- singing songs, talking about colors and letters; writing their names; listening to a story; rotating through educational activities; and having free play. Six child records and five staff records were reviewed. The Center has multiple staff with current certification in CPR and First Aid, as well as trained in Daily Health Observation. Thirteen medications and their authorization forms were reviewed and the center has two staff with current certification in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 9/14/22 and the most recent Health Inspection on file was dated 5/13/22. Lunch served was turkey and cheese wraps, green beans, pineapple, and milk. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-140-A Description: Based on review of six child records, the center did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance for one child. Evidence: The record for Child A, with a start date of 2/28/22, did not contain documentation of a physical. Plan of Correction: Requested physical from parent of child A; Parents will be required to present physical on first day of or prior to attendance.
Standard #: 8VAC20-780-160-C Description: Based on review of five staff records, the center did not ensure that a staff member obtained and submitted the results of a follow-up tuberculosis (TB) screening at least every two years from the date of the initial screening or testing. Evidence: The most recent TB screening or testing on record for Staff #4 was dated 11/11/19. Plan of Correction: Staff was instructed to have TB screen renewed. Verbal and written reminders
prior to expiration. Staff will be required to submit new screening results to be scheduled past expiration date.
Standard #: 8VAC20-780-80-A Description: Based on observation and review of documentation, the center did not ensure that for each group of children, a written record of daily attendance was maintained that documented the arrival and departure of each child in care as it occurred. Evidence: 1. At approximately 9:10am on 10/5/22, 10 children were observed in the three-year-old classroom. The attendance contained documentation of nine children present. 2. At approximately 9:20am, 13 children were observed in the two-year-old classroom. The attendance contained documentation of 12 children present. 3. At approximately 9:32am, four children were observed in the infant classroom. The attendance contained documentation of five children present. Plan of Correction: Admin will review attendance standards with all classroom staff.
Standard #: 8VAC20-780-510-P Description: Based on review of medication and medication authorization forms, the center did not ensure that when an authorization for medication expired, the parent was notified that the medication needed to be picked up within 14 days or the parent must renew the authorization. Evidence: An emergency medication was observed on site for Child B on 10/5/22. The corresponding medication authorization form contained documentation that the doctor?s authorization expired on 12/8/21. Plan of Correction: Admin will send request for updated authorization on three occasions prior to expiration. If updated authorization is not received medication will be returned to the family. Forms for updating authorization will be send out 30 days prior to expiration.
