Comments:
An unannounced renewal inspection was conducted on October 11, 2022 from approximately 12:30 pm to 2:15 pm. There were 118 children in attendance and a total of 20 staff present. 10 staff records and 10 children?s records were reviewed. All areas of the center were observed including classrooms, bathrooms, hallways, and the playground. The children were observed taking a nap while the licensing inspector was on the premises.

The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted. Fire drills were conducted monthly with the most recent on 9/27/22.

Violations were cited as a result of this inspection.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 10/17/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 8VAC20-780-240-A
Description: Based on review of staff records, and interview, the center failed to ensure that the Virginia Department of Education (VDOE) sponsored orientation course is completed within 90 calendar days of employment. Evidence: 1) The record for staff #6, date of employment 5/31/2022, and staff #8, date of employment 5/31/2022 did not have evidence of the VDOE orientation course. When interviewed the Director stated that they had not completed the course. 2) Staff #7, date of employment 5/31/2022, completed the VDOE course on 09/30/2022. The course must be completed within 90 days of employment.

Plan of Correction: 1. Steps to correct the noncompliance:
a. Follow up email was sent to the seasonal staff member who has not completed the training (staff #6) and she will be unable to work until it is completed.
b. For staff #8- time has been scheduled for the staff member to complete the training on Wednesday, October 26th.
2. Measures to prevent the noncompliance from occurring again:
a. Updated our onboarding process to include the 10 hour training as part of the new employee orientation. Staff will not begin work prior to completing this training.
3. Person(s) responsible for implementing each step and/or monitoring any preventative measure(s):
a. Assistant Director creates the schedules for orientation and will include this in all future schedules.

Standard #: 8VAC20-780-440-J
Description: Based on observation, and interview the center failed to ensure that there was at least 12 inches of space between occupied cots. Evidence: Four out of nine classrooms visited had several occupied cots that were less than 12 inches apart. The Director acknowledged that the cots were less than 12 inches apart.

Plan of Correction: 1. Steps to correct the noncompliance:
a. Staff has been retrained in the guidelines for cot distance during rest time. This training occurred on Thursday, October 13th and Friday, October 14th.
b. All cot charts were examined by an administrator to ensure they allow for 12 inches of space between occupied cots during these 2 days listed above.
2. Measures to prevent the noncompliance from occurring again:
a. All cot charts will be examined during monthly classroom observations done by the Assistant Director and Curriculum Specialists.
b. Director will complete monthly walk-arounds of all rooms to ensure cot distance is being followed during rest time.
3. Person(s) responsible for implementing each step and/or monitoring any preventative measure(s):
a. Assistant Director and Curriculum Specialist will complete the monthly observations.
b. Director will complete monthly walk-arounds during rest time.
