Comments:
An unannounced mandated inspection was conducted on 08/16/2022 from 1:35pm-4:00pm. There were 119 children supervised by 23 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in playing outside, group play, and napping. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review and staff interview, the center did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry, for an employee within 45 days of employment, from any state in which an employee has resided in the preceding five years, prior to the first day of employment.

Evidence: Staff #5 did not have results of an out-of-state child abuse and neglect registry, out-of-state sex offender registry check, and out-of-state criminal history check.

Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. In the Kittens classroom, a cleaning product was inside an unlocked cabinet.

2. In the Cardinals classroom, a cleaning product was hanging on an art rack above the sink.

3. In the Chicks classroom, a cleaning product was located above a sink adjacent to the diaper changing table.

Description: Based on staff interview, the center did not post a daily record for infants, who are awake and cannot turn over by themselves, the amount of time spent on their stomachs.

Evidence: Center management stated tummy time is practiced for each infant unable to turn over by themselves. However, the center does not document tummy time.
