Comments:
An unannounced monitoring inspection was conducted on 9/26/2022 from 12:21 pm-13:00 pm. There were 25 children supervised by 3 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and staff interview, the facility did not obtain a physical examination by or under the direction of a physician prior to a child's attendance; or within 30 days after the first day of attendance.

Evidence: Child #3 did not have documentation of a physical examination prior to or within 30 days after the child?s first day of attendance.

Plan of Correction: I will contact mom about it today, 9/26/2022.

Standard #: 8VAC20-780-60-A-8
Description: Based on medication review and interview, the facility did not ensure that there is a written allergy care plan for each child with a diagnosed food allergy.

Evidence: Child #6 did not have a written allergy care plan for diagnosed food allergies.

Plan of Correction: Will ask parents about it this afternoon, 9/26/2022.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence: Two containers of disinfectant wipes were located on a shelf next to a window.

Plan of Correction: Corrected during inspection.
