Standard #: 8VAC20-780-60-A Description: Based on a review of children's records, it was determined two records do not contain all required information.
1 - Child record B does not include information for the parent's home phone number.
2 - Child record C does not include information for each parent's work phone number.
Plan of Correction: Each parent will be asked to NA those sections. We have each parent's cell number, with directions to use the cell for all calls.
