Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program. A monitoring inspection was initiated on October 12, 2022 and concluded on October 13, 2022. There were 39 children present, ranging in ages from 5 to 9, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-K
Description: Based on review of center documentation and staff interviews, the licensee did not ensure the center shall develop written procedures for abusive head trauma.

Evidence: Staff #1 confirmed there was not a policy available for review related to head trauma.

Plan of Correction: Area Manger has emailed out the head trauma policy and it is available for review in the licensing binder.

Standard #: 8VAC20-780-60-A
Description: Based on review of 5 children?s records and staff interviews, the licensee did not ensure that each child shall maintain and keep at the center a separate record for each child enrolled which shall contained the required information.

Evidence: Staff #1 confirmed the following missing information for 3 out of 5 of the children?s records reviewed.
1. Child #1 did not have a home addresses or work location and phone number for one of the parents listed.
2. Child #2 did not have a home addresses or work phone number for one of the parents listed.
3. Child #3 did not have in the record documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Child #3, first date of attendance 8-29-2022

Plan of Correction: Child #1's file has been updated with the missing information as of 10-12-22.
Child #2's file has been updated with the missing information as of 10-13-22.

Standard #: 8VAC20-780-510-P
Description: Based on review of documentation and staff interviews, the licensee did not ensure when an authorization for medication expires, the parent shall be notified the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Evidence: Child #6 had a medication (albuterol) the medication authorization signed by the physician expired on 8-9-2022. The medication had not been sent home to the parents or disposed of as required.

Plan of Correction: Child #6's medication was sent home along with MAT paperwork to be updated on 10-13-2022. Program manager will check medication monthly to ensure medication forms are updated and current and expired medication is sent home in the required timeframe.
