Technical Assistance:
Inspector discussed the benefits of using the most recent model forms for emergency medication, child information, and emergency drills.

Comments:
A monitoring inspection was initiated on 10/6/2022 and concluded on 10/6/2022. There were a total of 31 children with 5 staff members present. Children were observed during lunch time, engaged in outdoor play time, and preparing for naptime. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on file review, an employee did not have a completed sworn statement.

Evidence: Staff #1 had a sworn statement that expired 2/3/2022.

Plan of Correction: Center had staff member complete a new sworn statement.
