An unannounced monitoring inspection was conducted on 10/13/2022 from 9:45am to 12:45pm. There were 66 children ages 5 months to age 5 years old supervised by 12 staff. The physical plant, outdoor playground area, programming, menus, 5 staff records, 5 children?s records, 16 medications with corresponding authorization records, emergency drills, and emergency supplies were reviewed. Children were observed participating in art, reading, blocks and dramatic play. Diapering and handwashing procedures were also observed. There was an adequate number of staff present with current certification in MAT, CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-770-60-B Description: Based on review of 5 staff records, the center did not obtain documentation of a completed sworn disclosure statement for each staff prior to date of hire.
Evidence: The record for Staff #1 (date of hire 6/29/22) did not contain documentation of a completed sworn disclosure statement prior to the date of hire. The sworn disclosure statement was completed on 6/30/22.Plan of Correction: Prior to date of hire, staff will complete sworn disclosure.
