Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on record review and interview, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #6, Child #7, Child #8, and Child #10 have emergency medication on site and are diagnosed with a food allergy. Child #6's, Child #7's, Child #8's, and Child #10's records did not contain documentation of a written care plan .
2. Staff #5 confirmed that a written care plan was not obtained for each child diagnosed with a food allergy.

Plan of Correction: The director has downloaded and printed the FARE form from the internet. The forms will be given to the parents to have the physician to complete. Once the forms have been completed, the children's records will be documented.

Standard #: 8VAC20-780-550-G
Description: Based on review of documentation, documentation was not maintained of emergency evacuation, shelter in-place, and lockdown drills.
Evidence:
1. Documentation of emergency evacuation, shelter in-place, and lockdown drills did not contain required information. The documentation was missing the identity of the person conducting the drill, method used for notification of the drill, and problems encountered, if any.

Plan of Correction: The center has printed the model forms from the department and will use the model from moving forward.
