A renewal inspection was conducted on 10/21/2022. There were 109 children present, ranging in ages from infants to 5 years, with 20 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 9 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Evidence:
The injury record for child 8, dated 10/19/2022, did not include the date the parent was notified of the child's injury.Plan of Correction: The facility responded with the following during the exit interview:
The parent of child 8 was notified of the child's injury and signed the injury record. All children's injury records will be carefully checked to ensure that all required documentation is complete.
