Technical Assistance:
Technical assistance was offered related to staff communication and implementing a system to ensure proper care of children when staff transitions take place.

Comments:
An unannounced complaint inspection was conducted on 10/13/22 from 11:40 am to 12:30 pm. The complaint was related to the care and supervision of a child. 3 staff members were interviewed. Documents reviewed included the following: Child Supervision Record for 10/13/22 for the classroom; 2 staff records and 1 child's record. The evidence gathered during the investigation supported the allegations of non-compliance with standards or law and violations were issued. Description: Based on staff interviews, staff did not ensure care and protection of a child while supervising them.
Evidence:
1. On 10/4/22, Staff # 1 and Staff # 2 took the 3?s class out on the playground at approximately 10:40 am and came back into the classroom at approximately 11:00 am.
2. Staff # 1 stated that while on the playground, Child # 1 (3-years old) jumped in a small puddle of water, left from the rain the day before. Staff # 1 noticed that Child # 1?s pants had gotten wet from the water up to his knees.
3. When they returned back to the classroom, it was time for Staff # 1 to leave on lunch break. Staff # 1 told Staff # 2 to change Child # 1, but did not specify that Child # 1?s clothing had gotten wet. At approximately 11:03 am, Staff # 1 left on break.
4. Staff # 2 stated that when Staff # 1 mentioned Child # 1 being wet, Staff # 2 thought that Staff # 1 was referring to Child # 1?s pull up, and did not notice that Child # 1?s pants were wet. Staff # 2 stated that they got busy with handwashing and fixing lunch, and did not get a chance to check Child # 1?s pull up.
5. After lunch, the children got on their cots for nap time. Staff # 2 stated that they were going to check on Child # 1 but by that time Staff # 1 returned from break.
6. Staff # 1 returned from break at approximately 12:03pm and saw that Child # 1 was asleep on his cot, covered by his blanket, and assumed that he had been changed.
7. Staff # 1 stated that at 2:35 pm Child # 1 woke up. When helping him up, Staff # 1 realized that Child # 1?s pants were still wet. Staff # 1 stated that ?Child # 1?s pants, socks and shoes were soaking wet?, and when he got up from his cot, his cot cover was also wet.
8. Child # 1 was in wet pants, socks and shoes from approximately 11:00 am to 2:35pm.

Plan of Correction: We conducted a training on communication during PDD.
