Comments:
An unannounced monitoring inspection was conducted on-site October 425 2022. The director was available during the inspection. There were 43 children present, ranging in ages from 3 months to 5 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22.1-289.058
Description: Based on observation and staff interview on October 25, 2022, the center failed to ensure that each building that was built before 2015 and that house a day program that is licensed and serves preschool-age children shall be equipped with at least one carbon monoxide detector.
Evidence: 1. Pre-school aged children were observed in care. 2. Staff #5 confirmed that both licensed buildings of the center did not have a carbon monoxide detector.

Plan of Correction: I plan to email Richmond so that are aware centers may not be aware of the Code of Virginia or this law requirement as it is not in the standards(I was completely unaware). This law is not in the standards dated 10-13-21 but centers are to know this is a requirement. 2 detectors were installed within 2 hours of the completion of the inspection.

Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records and interview on October 25, 2022, the center failed to ensure that each staff record reviewed contained a central registry finding within 30 days of employment.
Evidence: 1. The record of staff #2, hired 7/12/22, did not contain central registry results. 2. Staff #5 confirmed the central registry results were not complete.

Plan of Correction: The central registry background check had been returned twice for corrections and has not been received. An email went out an hour after the inspection to check on it.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview on October 25, 2022, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1. There was bleach water and Lysol spray in an unlocked cabinet, accessible to children, in the 3 year old classroom. The label on the Lysol stated "keep out of reach of children" and "caution." Staff #5 confirmed the cabinet was unlocked. 3
2. In the bathroom used by children, there was an unlocked cabinet that contained Tide detergent and there were two refill containers of disinfectant cleaner on a shelf above the washer and dryer. The label on the Tide stated "keep out of reach of children" and "caution" The label on the disinfectant cleaner stated "keep out of reach of children" and "danger." Staff #5 confirmed the cabinet was unlocked and that the disinfectant cleaner was always kept on the shelf.

Plan of Correction: Two cabinets were unlocked at time of walk through that contained hazardous materials. Both cabinets were locked in sight of the inspector and Staff #5 spoke to all staff about the importance of locked cabinets.
