Comments:
An unannounced inspection was initiated on 9/29/22 with a local agency in response to a self-report by the center regarding an allegation of a forbidden action, rough handling, of a child in care. Interviews were conducted, documentation was reviewed, and one staff record was reviewed. The inspection resulted in a violation. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice and return to me by close of business on 10/28/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-410-1
Description: Based on interviews and review of documentation, the center did not ensure that a forbidden action, rough handling of a child in care, did not occur. Evidence: Interviews and documentation revealed that on 9/22/22, between 4:30pm and 5:30pm in the School Age Classroom, Child A (8-years-old) became upset when another child sat in the seat they had been sitting in. Staff #1 approached Child A to get them to move to another area to talk about the situation. When Child A did not follow, Staff #1 put their hands on Child A?s upper arms and went to move Child A to the other area. Child A said, ?Ow!? and began to cry and tell Staff #1 that Staff #1 had hurt their arm. A small bruise was later seen on Child A?s left upper arm.

Plan of Correction: The school manager self-reported the incident to all outside agencies. The staff member was immediately placed on administrative leave. Upon completion of the investigation, the staff member's employment was terminated.
