Comments:
An unannounced monitoring inspection was conducted on 10/26/2022 from 10:45 a.m. to 12:40 p.m. There were 53 children present, ranging in ages from 3 to 5 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on review of records and interview, the center failed to ensure that staff obtained a tuberculosis (TB) screening at least every 2 years.
Evidence:
1. Staff #5?s TB screening on file was dated 9/3/2020. Staff #6 confirmed that Staff #5 did not have an updated TB screening on file.
2. Staff #3?s TB screening on file was dated 10/6/2020. Staff #6 confirmed that Staff #3 did not have an updated TB screening on file.

Plan of Correction: We will have them completed by 11/15/2022.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, the center failed to maintain a written list of all children?s allergies, sensitivities and dietary restrictions.
Evidence:
1. In each classroom there was a list of allergies and dietary restrictions for the children in that classroom only.
2. Staff #6 confirmed that the classrooms only had a list for the children in that classroom and that the lists did not include all children.

Plan of Correction: Allergies will be posted by the end of the day.
