Technical Assistance:
Consultation was given on model forms including a form to list allergies, food sensitivities and preferences.

Comments:
An unannounced monitoring visit was conducted during the after school session of extended day. 21 children were present with two staff which is within ratios for supervision. Children were observed finishing snack bought from home, transitioning to the outside for play time and in the cafeteria for games, homework and reading. Reviewed staff information, a sample of children's records, an emergency medication, emergency preparation and supervision. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on record review, a sample of children's records were incomplete.
Evidence:
Child #1 was missing the physician phone number and two emergency contact addresses.
Child #2 was missing parent work information and physician information.
Child #3 and Child #4 were missing emergency contact information

Plan of Correction: Information will be requested from each parent and guardian to complete the missing inforamtion.

Description: Based on observation, an area of the extended day space has not been maintained in a clean, safe or operable condition.
Evidence:
There are 5 spots along the cafeteria walls wee the paint is peeling away.
