Discussed with school administrative staff: potential for 1-on-1 training for admin staff for file requirements

Comments:
An in person investigation was begun today in response to an allegation being received in our office. Additional standards were reviewed to consider this inspection a mandated visit. Based on a review of documentation, the preponderance of evidence supports the allegation that a written record of injuries that children received while in care was not maintained and/or that injuries were not reported to parents. The complaint is valid.
Violations were discussed with the Administrative staff. See Violation Notice for non-compliances. Staff and child record violations were confirmed during the exit interview with the staff person responsible for recordkeeping.
Emergency supplies, First Aid supplies. Emergency evacuation drills, lunch boxes labeled/dated, handwashing and bathroom supplies were all in compliance today.

Description: Based on a review of injury/accident report documentation and a child record, the center did not maintain a written record of children?s serious and minor injuries in which the entries are made the day of the occurrence.
Evidence:
No written record was maintained of injuries occurring at the center for a child.
1. On 7/26/22 Parents were notified via the ?Bright Wheel? app of a head injury for their child. There is no written report documenting available documenting; staff present, any future action to prevent recurrence, staff and parent signatures
2. A written record of an injury was not maintained at the center for an Injury, that occurred on 7/14/22.
3. On 6/9/22 the center was contacted to inquire about an injury the child received at the center (face scratches). On 6/10/22 the center responded stating ?Sure, we will investigate this issue today?. There is no written report for this injury.
