Comments:
A monitoring inspection was conducted on 10/27/2022. There were a total of 20 children with 4 staff members present. Children were observed during lunch time, engaged in group activities and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-160-C
Description: Based on a review of staff files, there was not a TB test available at least every two years from the date of the first initial screening or testing.
Evidence:
Staff #2 had a TB test dated 8/23/2019 on file during the inspection.

Plan of Correction: Staff has obtained a current TB test and results.

Standard #: 8VAC20-780-40-N
Description: Based on observation, there was not a written playground safety procedure available.
Evidence:
There was not a written playground safety procedure available during the inspection.

Plan of Correction: The playground safety procedures have been located and updated. The procedures are placed in the licensing binder.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method.
Evidence:
There were unlocked Clorox wipes above the first aid cabinet in the 2 year old room. There were unlocked cleaning supplies (fabuloso, febreeze, & simonize) unlocked underneath the sink in the hallway children's bathroom. There were unlocked cleaning supplies in a lower cabinet in the 4 year old room.

Plan of Correction: Staff has been counseled. Cleaning products have been placed and locked in cabinets.
