Comments:
An unannounced monitoring inspection was conducted on-site October 27, 2022. The assistant director was available during the inspection. There were 24 children present, ranging in ages from 4 months to 5 years, with 11staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, the center failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence: The toddler classroom had an unlocked cabinet under the diapering station containing bleach water. Documentation of diaper changes showed the last diaper change occurred at 1:58pm and staff confirmed the time. The unlocked cabinet containing the bleach water was observed at 2:10pm.

Plan of Correction: The lead teacher will ensure all hazardous substances are locked away beneath the changing table. The assistant director will monitor for compliance.

Standard #: 8VAC20-780-510-G
Description: Based on observation, the center failed to label each medication with the child's name, the dosage amount, and the time to be given.
Evidence: A bottle of children's Motrin and Fever All was not labeled with the child's name, dosage amount, and the time to be given.

Plan of Correction: All medications will be labeled with the name, dose and time. The director will monitor for compliance.
