An unannounced monitoring inspection was conducted on 10/12/22 at 10:00am to 12:15pm to investigate a self-reported incident regarding the care and protection of children in the infant rooms after a child was fed breast milk that belonged to another child. Interviews were conducted with 4 staff members. Attendance documentation, 2 child files, and 2 staff files were reviewed. All 3 infant classrooms were observed. Standard #: 8VAC20-780-340-A Description: Based on interviews, the center did not ensure the care, protection and guidance of all children in care.
Evidence: On 10/7/22, Child #1 was fed Child #2?s bottle containing breast milk. Child #2 had been dropped off in Child #1?s classroom. Child #2 was then moved to their normal classroom when their teacher arrived for the day. Not long after the transition, Staff #1 began feeding Child #2. When it was time to feed Child #1 in the other classroom, Staff #2 noticed that Child #1?s bottles were missing and that Child #2?s bottles were left in the classroom. At that time, Staff #2 called the adjoining classroom to inform of the missing bottles and learned that Child #1, had just finished consuming the entire bottle of breast milk that belonged to Child #2. When interviewed, Staff #1 stated they didn?t read the label on the bottle before feeding it to Child #1.?Plan of Correction: Retrain all team members regarding proper call and feeding procedures, ensure all bottles are checked before feeding.
Standard #: 8VAC20-780-500-B Description: Based on observation, the center did not ensure that disposable diapers were disposed in a storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence: The foot-operated storage system for disposal of diapers was not able to be operated by foot causing the staff to touch the exterior surface of the trash can with their hand or a soiled diaper to open the trash can.Plan of Correction: New trash can purchased at time of inspection.
Standard #: 8VAC20-780-530-A Description: Based on documentation review and interview with staff, the center did not have at least one staff member in each classroom or area where children are present that had obtained first aid and cardiopulmonary resuscitation (CPR) certification.
Evidence: The file for Staff #1 (Date of Hire 05/23/22) did not contain documentation for completion of first aid and cardiopulmonary resuscitation (CPR) certification. Staff #1 confirmed she is regularly staffed in the Infant C classroom as the only staff member.Plan of Correction: Corrected immediately after inspection.
Standard #: 8VAC20-780-560-G Description: Based on observation, the center did not ensure that food brought from home was clearly dated and labeled in a way that identifies the owner.
Evidence: In the Infant C classroom, Child #3 had brought food from home that was stored in the refrigerator and was not dated.Plan of Correction: Ensure all food is labeled with the date. Corrected during inspection.
Standard #: 8VAC20-780-570-E Description: Based on observation, the center did not ensure that prepared infant formula was dated and labeled with the child?s name.
Evidence: In the Infant A classroom, Child #4 had a bottle of infant formula that was stored in the refrigerator and was not dated.Plan of Correction: Ensure all bottles are labeled with the date. Corrected during inspection.
