Comments:
An onsite renewal inspection was initiated and concluded on October 26, 2022. There were 71 children present ages two through six years old with nine staff. The inspector conducted a walking tour of the building inside and outside. Children were observed in teacher and self-directed activities including but not limited to outside play, learning center play activities, lunch, nap, restroom routines and transitions. Children and staff records were reviewed. Required postings, emergency evacuation drills, annual inspections, medication, first aid kit and emergency equipment were reviewed.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and discussed with the center representative during the exit interview.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review and interview the center failed to ensure that each employee submits to pringerpinting prior to employment.
Evidence:
1 - The records for Staff #'s 2 - 6 contain documentation of fingerprint background completed after the documented date of employment.
a. Staff #2 has a documented date of hire as September 9, 2022 and a fingerprint background check dated September 16, 2022,
b. Staff #3 has a documented date of hire as May 17, 2022 and a fingerprint background check dated May 19, 2022,
c. Staff #4 has a documented date of hire as March 1, 2022 and a fingerprint background check dated March 4, 2022.
d. c. Staff #5 has a documented date of hire as March 1, 2022 and a fingerprint background check dated March 2, 2022.
e. c. Staff #6 has a documented date of hire as March 1, 2022 and a fingerprint background check dated March 2, 2022.

Plan of Correction: Going forward center management will ensure that fingerprints are obtained prior to employment. Hire dates will be documented as of the first time a staff clocks in for payment of services.

Standard #: 8VAC20-780-160-A
Description: Based on record review and interview the center failed to ensure that each staff member submits documentation of a negative tuberculosis screening.
Evidence:
1 - The record for Staff #2 does not contain documentation of a negative tuberculosis screening.
a. Staff #2 has a documented date of hire as September 9, 2022.
2 - Staff #1 confirmed that there are not any tuberculosis screening results in the record of Staff #2.

Plan of Correction: Going forward center management will ensure that all tuberculosis documentation contains required screening result information prior to the documentation being placed in the staff record.

Description: Based on record review and interview the center failed to ensure that each staff contains all required information.
Evidence:
1 - The record for Staff #5 did not contain documentation of education.
a. Staff #5 has a documented date of hire as March 1, 2022.
2 - Staff #1 confirmed that there is not any education documented in the record of Staff #5.

Plan of Correction: Center management will ensure that documentation of education is placed in all staff records at the point of hire.
