8VAC20-780 Staffing and Supervision

Comments:
A non-mandated inspection was initiated on August 25, 2022 and concluded on October 5, 2022. A self-reported incident was received in the Office of Child Care Health and Safety (Valley Office), on August 22, 2022 regarding sight and sound supervision and care and protection. Interviews were conducted on 08/25/2022 and 09/09/2022 in conjunction with a local agency. One child record, two staff records, five interviews, and observations of the classroom and bathroom procedures were conducted.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on staff interviews, the center failed to ensure the children?s care, and protection at all times.

Evidence:
1. Interviews with Staff #2, #3, and #4, stated that on 08/23/2022 from approximately 11: 50 am to 12: 00 pm Child #1, age 3, was left outside on or near the playground area.
2. During an interview, Staff #4 confirmed that they were unaware that Child #1 was left outside.
3. Interview with Staff #3 revealed that two unknown women knocked on the door and were returning Child #1 to the facility.
4. E. Market Street a four (4) lane road to go Downtown Harrisonburg is approximately 60 feet from the stairs of the facility.

Plan of Correction: Please review the corrective action for the situation that occurred on August 22, 2022. The Harrisonburg Rockingham Child Day Care Center is very distraught and disturbed that this situation even occurred. We work very hard to make sure that all staff members are trained before working with children. One of the training components is explaining the importance and requirement of knowing how many children are in their care at all times. All staff members are reminded of this requirement after working with children and periodically throughout each year.
The two staff members involved in the situation are no longer employed with our program. All staff members have been reminded of the requirement that head counts of all children must be taken before, during and after each and every transition that occurs throughout the day. All staff members, regardless of position or title, are required to know how many children are in their care at each moment of the day. If two staff members are working together through a transition, both staff members are required to know how many children are in their care.
Continual reminders will be given and continual monitoring of this requirement will happen with the Executive Director and the office manager to make sure that this situation does not ever happen again.

Description: Based on staff interviews, the center failed to ensure that staff maintained sight and sound supervision of children under 10 years of age at all times.

Evidence:
1. Interviews with Staff #2, #3, and #4, stated that on 08/23/2022 Child #1, age 3, was out of sight and sound supervision with staff from approximately 11:50 am to 12:00 pm.
2. Interview with Staff #3 revealed that two unknown women knocked on the door and were returning Child #1 to the facility.
