Technical Assistance:
Discussed monitoring of playground equipment and classroom furniture for rusting metal or broken/splintering wood. The updated 2021 CDC standards were discussed and the Director was shown where to find the requirements for updating the Emergency plan.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 11:00am and 12:30pm. There were 4 classrooms observed with a total of 37 children with 10 staff within the supervision guidelines. The children were observed having circle time, playing on the playground and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, 2 out of 3 staff did not submit documentation of a negative tuberculosis screening that was completed within the last 30 calendar days of the date of employment.

Evidence:
1. Staff #1 (doh: 8/8/2022) had a tuberculosis screening on file that was dated 5/28/2021.
2. Staff #3 (doh: 8/9/2022) had a tuberculosis screening on file that was dated 5/22/2022.

Plan of Correction: I will have the staff get new ones in the next 2 weeks and will make sure new staff that are hired have TB screenings that have been completed within the last 30 calendar days of the date of employment.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.

Evidence:
1. In the Toddler classroom, there was peeling tan colored paint around the bulletin board in the back of the classroom.

Plan of Correction: We will contact our handyman today to fix this.

Standard #: 8VAC20-780-550-A
Description: Based on review of emergency procedures, the Emergency Preparedness Plan has not been updated annually and does not address staff responsibility and facility readiness with respect to emergency evacuation and relocation.
Evidence
The current Emergency Preparedness Plan was last updated in April 2003. The plan is still not complete and does not contain all required components. Staff phone numbers, responsibilities and facility readiness in regards to emergencies is not complete.

Plan of Correction: We will update the emergency plan and add all of the required information that is listed in the CDC standards.

Standard #: 8VAC20-780-570-E
Description: Based on observation, all prepared infant formula was not refrigerated, dated and labeled with the child's name.

Evidence:
1. There were 3 bottles labeled with Child #4's name in the infant room refrigerator that did not have the date on them.

Plan of Correction: We will make sure all bottles have the date moving forward.
