Standard #: 22.1-289.035-B-4 Description: Based on record review, the facility did not ensure a required out of state child abuse and neglect background check request is completed by the end of the 30th day of employment.
Evidence:
1. Staff #4's date of hire is 02/21/22 and the sworn statement indicated that staff member lived in GA in the last 5 years.
2. There is no out of state child abuse and neglect background check results in Staff #4's file.
3. Administration confirmed that there is no out of state background check in Staff #4's file.Plan of Correction: We will make sure that the out of state background checks are requested and received within a timely manner.
Standard #: 8VAC20-780-160-A Description: Based on record review, the facility failed to obtain documentation of a negative tuberculosis screening at the time of employment.
Evidence:
1. Staff #2's date of hire is 08/10/2022 and the date of the TB screening is documented as 08/13/2022.
2. Staff #3's date of hire is 08/10/2022 and the date of the TB screening is documented as 08/12/2022.
3. Administration confirmed the dates of hire and the dates of the TB screenings.Plan of Correction: I will make sure that staff has documentation of TB screening or test prior to employment.
