Comments:
An unannounced monitoring visit and facility tour was conducted on 11/7/2022. At the time of inspectors' arrival there were 16 infant to preschool aged children in care with 4 staff members. Children were observed interacting with staff and nap. A sample of 5 children's records and 4 staff records were reviewed. Injury reports were also reviewed. Assistant Director reported that medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review and interview it was determined the center did not obtain a copy of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years for each staff member.
Evidence:
1. Staff #3 reported they had lived in Florida within the past five years. The record for staff #3(hired 6/22/22) did not contain a copy of a sex offender registry check, and a search of the child abuse and neglect registry for the state of Florida.
2. Staff confirmed these checks had not been completed for staff #3 through Florida.

Plan of Correction: Out of state background checks will be requested.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview it was determined the center did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There were unlocked Clorox spray on a high shelf in the 2's, 3-4's and infant classrooms.
2. Staff confirmed that there were unlocked Clorox sprays in the classrooms.

Plan of Correction: All chemicals will be locked.

Standard #: 8VAC20-780-550-D
Description: Based on record review and interview it was determined the center did not implement a monthly practice evacuation drill.
Evidence:
1. There was no monthly evacuation drill practiced in October 2022.
2. Staff confirmed there was no drill practiced in October.

Plan of Correction: Director will ensure evacuation drills are completed monthly.

Standard #: 8VAC20-780-550-P
Description: Based on record review it was determined the center did not ensure the written record of children's serious and minor injuries contained all of the required information.
Evidence:
1. The injury report for child #6 dated 10/21/22 did not contain the time of the incident.
2. The injury report for child #7 dated 10/21/22 did not contain the time and method of parental notification and the future action to prevent recurrence.
3. The injury report for child #8 dated 10/20/22 did not contain witnesses, the date of parental notification and the future action to prevent recurrence.
4. The injury report for child #9 dated 10/11/22 did not contain the future action to prevent recurrence.
5. The injury report for child #10 dated 10/07/22 did not contain the future action to prevent recurrence.

Plan of Correction: Staff will be retrained on completing injury reports.

Standard #: 8VAC20-780-570-E
Description: Based on observation and interview it was determined the center did not ensure that prepared infant formula was refrigerated, dated and labeled with the child's name.
Evidence:
1. There was a prepared bottle in the refrigerator that had no name or date on it.
2. Staff identified the bottle and confirmed it was not labeled.

Plan of Correction: Staff will ensure all bottles are properly labeled.
