An unannounced monitoring inspection was conducted on 10/26/2022 from 7:30am to 9:30am. There were 33 children ages 5 years old to age 11 years old supervised by 3 staff. The physical plant, outdoor playground areas, programming, menus, 3 staff records, 3 children?s records, 2 medications with corresponding authorization records, emergency drills, injury reports, and emergency supplies were reviewed. Children were observed participating in small group play, art, legos and snack. Sanitizing and handwashing procedures were also observed. There was an adequate number of staff present with current certification in MAT, CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-80-A Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.
Evidence: During inspection in the Before and After school classroom, there were 33 children present, but the written record reflected there were 30 children present.Plan of Correction: Printed new roll sheet for students.
Standard #: 8VAC20-780-270-A Description: Based on observation, the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence: On the basketball court playground area, large holes are present in the fencing and they have been covered by sharp metal patches that could snag a child?s skin.Plan of Correction: Reported to maintenance for repair.
