Comments:
A monitoring inspection was conducted on November 2, 2022 from 11 am until 12:30 pm. There were fifteen children present, ranging in ages from three to five with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and two staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 8VAC20-780-70
Description: Based on observation and interview, program did not ensure that emergency contact information is kept at the center.

Evidence:
1. During tour of classroom, was unable to find staff emergency contact information.
2. Staff #2 confirmed that they did not have access to other staff member's emergency contact information.
3. Emergency contact information was available in the staff members' files at headquarters.

Plan of Correction: Emergency contact information for staff was emailed to classroom.

Standard #: 8VAC20-780-500-A
Description: Based on observation, program did not ensure that staff wash their hands before helping children with feeding.

Evidence:
1. Staff #1 sat down at the table with four children. Staff # 1 asked Child #2 if he wanted to drink his milk and proceeded to open it for him. When Child #1 spilled his milk on the floor, Staff #1 got up to get paper towels so that Child #1 could clean up the spill. Staff #1 also picked up the trash can to bring it closer to the spill for paper towel disposal. After this, another child at the table requested help with their milk carton. Staff #1 opened the milk for the child.
2. Staff #1 did not wash hands before sitting down at the table with the children, nor before helping the children with their respective milk cartons.

Plan of Correction: Staff reminded of food policy.
Staff reminded of handwashing.
