Comments:
An unannounced monitoring inspection was conducted on 11/14/2022 from 9:45am to 12:00pm. At the time of entrance, 108 children were in care with 17 staff members present. Children were observed playing with balls in the sports classroom, playing outside, working on a leaf stamp art project, listening to a teacher read a book, washing hands, playing with toys, having diaper changes, being held, playing with toys on the floor and sleeping in cribs. Interactions between the children and staff were positive. The site was organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, medications, allergy plans, the physical space, evacuation drills, and attendance records were reviewed. Description: Based on review of staff records, the center did not obtain the central registry findings within 30 days of employment.
Evidence:
1. Staff #3 (Date of hire 8/01/2022) did not have the central registry findings within 30 days of employment. The center had documentation that it was returned for revision.

Plan of Correction: Contacted agency to updated requested info.

Standard #: 8VAC20-780-50-A
Description: Based on observation, children's records were not treated confidentially.
Evidence:
1. In the Elephants classroom, the allergy list was in a sleeve on the bookshelf with confidential allergy information not covered and in view to others.

Plan of Correction: The allergy list was relocated to it's original confidential spot.

Description: Based on review, the center did not obtain a complete record for a staff member.
Evidence:
1. Staff #3 (Date of hire 8/01/2022) was listed as a teacher, but did not have documentation to demonstrate that the individual possesses the education required by the job position.

Plan of Correction: Received the employees diploma.

Standard #: 8VAC20-780-270-A
Description: Based on observation, equipment of the center were not maintained in a safe and operable condition.
Evidence:
1. In the Elephant classroom, a blue chair seat cushion had a hole in the fabric on the right side of the cushion.
2. In the Elephant classroom, two hooks were taped and protruding upwards on a shelf.
3. In the Bear Cubs classroom, a flue sofa had a hole in the fabric on the back of the cushion.
4. In the Bunnies classroom, three soft steps (yellow, green and purple) were coming apart at the seems/corners and creating small holes in the fabric.

Plan of Correction: The items will be removed and fixed or replaced.

Description: Based on review of children's records, the center did not obtain at least annually parent confirmation that the required information in the child's record is up to date.
Evidence:
1. Child #5 (start date 3/21/2021) did not have documentation that the parent confirmed the required information in the child's record was up to date. The last signature was listed 3/21/2021.

Plan of Correction: The child's annual contact info has been updated.

Standard #: 8VAC20-780-560-G
Description: Based on observation, food brought from home was not all clearly dated.
Evidence:
1. In the Hippos classroom, five children had lunch boxes in their cubbies that were not dated with today's date.

Plan of Correction: The children's lunchboxes were dated.
