An unannounced monitoring inspection was conducted 10/25/2022 from 10:20am-12:05pm. There were 34 children directly supervised by 4 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were inspected. The center reported no medications were on site. Children were observed playing with lincoln logs, legos, and trains. Child were also observed participating in writing and math activities as well as completing a craft. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. There was an adequate number of CPR/First Aid and Daily Health Observation trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-4 Description: Based on record review, the center did not obtain the results of a criminal history record search or request a search of the child abuse and neglect central registry from all states prior to hire in which staff have resided within the last 5 years.
Evidence:
1. Staff #2 (Date of Hire: 9/6/2022) has resided in Pennsylvania, California, and Maryland within the last 5 years. The center did not obtain the results of a criminal history record search for Pennsylvania or California and did not request a search of the child abuse and neglect central registry from Pennsylvania, California, or Maryland for Staff #2.
2. Staff #3 (Date of Hire: 9/8/2022) has resided in Pennsylvania within the last 5 years. The center did not obtain the results of a criminal history record search for Pennsylvania and did not request a search of the child abuse and neglect central registry from Pennsylvania for Staff #3.Plan of Correction: Did not live in PA within 5 years. All other states have yet to send us results. PA will send other results by end of next week.
Standard #: 8VAC20-780-70 Description: Based on record review, the center did not ensure that all staff records contained all required information.
Evidence: Staff #3 (Date of Hire: 9/8/2022) did not have documentation of completed orientation available at the time of inspection.Plan of Correction: Hall info but was unable to retrieve because we are under staffed.
Standard #: 8VAC20-780-260-B Description: Based on record review and staff interview, the center has not obtained an annual health inspection.
Evidence: The center has not had a health inspection within the last year. The most recent health inspection on file was completed 10/11/2019.Plan of Correction: Called 10/26/22. Logged conversation.
Standard #: 8VAC20-780-280-B Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence: Disinfectant sprays and a glass cleaner were stored unlocked on a shelf near the teacher's desk in the Kindergarten classroom.Plan of Correction: Locked. Teachers told items must be locked not store out of reach.
