The licensing inspector initiated an unannounced monitoring inspection in response to a self-reported incident regarding a child's injury that was received by the Department on April 5, 2022. The investigation was initiated on April 5, 2022 and completed on July 11, 2022. On April 6, 2022, two licensing inspectors conducted an unannounced inspection from approximately 10:17am to 12:40pm. Interviews were completed, observations were made at the center, and documentation was reviewed. Additional documentation was received electronically on April 18, 2022. On May 23, 2022, the inspector was on site from approximately 10:00am to 3:50pm conducting additional interviews and observations. No violations were found related to the self-report.
