Comments:
An unannounced monitoring inspection was conducted on 11/15/2022. There were 20 children, ages 3 years-5 years and 3 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medication was observed onsite. A total of 3 children?s records, 3 staff records and 2 medications were reviewed. The children were observed during center time and learning time. The inspector arrived for the inspection at 9:56am and departed the center at 11:56am.

Discussion on standards related to shaken baby/abusive head trauma policies. Discussion on standards related to carbon monoxide detectors. Discussion on first aid and emergency supplies.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 22.1-289.035-A
Description: Based on the review of three staff?s records, the center failed to ensure that 5 year repeat results for Central Registry findings were obtained.

Evidence: Staff #3 Central Registry results were dated 6/13/2017 and on the day of the inspection did not have the 5 year repeat results documented.

Plan of Correction: Provider will submit Central Registry results.

Standard #: 8VAC20-780-140-A
Description: Based on the review of three children?s records, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Evidence: Child #1 first date of attendance was 8/31/2022 and on the day of the inspection did not have the required physical examination record documented.

Plan of Correction: Provider will obtain physical examination from parent.

Standard #: 8VAC20-780-160-A
Description: Based on the review of three staff?s records, the center failed to ensure that documentation of a tuberculosis screening shall be submitted at the time of employment and prior to coming in contact with children.

Evidence: Staff #2 date of hire was 8/17/2022 and on the day of the inspection did not have the required tuberculosis screening documented.

Plan of Correction: Provider will obtain tuberculosis screening results.
