Comments:
An unannounced renewal inspection was conducted on Monday, November 7, 2022 from 9:00am to approximately 11:15am. There were a total of 65 children in care in the direct care of 13 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, kitchen, hallways, playgrounds, and bathrooms were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. Medication is administered when required, but no medications were on site. During the inspection, six children's records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of six staff records, the center did not ensure one staff member submitted documentation of a negative tuberculosis screening within the required timeframe.

Evidence: The tuberculosis screening in the record for Staff #6, employed 06/06/22, was dated 08/10/22. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.

Plan of Correction: Corrected. In the future, tuberculosis screening or testing will be required to be provided on the first day.

Standard #: 8VAC20-780-60-A
Description: Based on a review of six children's records and interview, the center did not ensure four children's records contained the required information.

Evidence: 1) The following records did not contain a work phone number for each parent who has custody: Child #2 (enrolled 08/29/22), Child #3 (enrolled 08/29/22), and Child #6 (enrolled 10/31/22).

2) The record for Child #1, enrolled 06/24/18, did not contain documentation that the parents were provided at least semiannually in writing information on their child's development, behavior, adjustment, and needs. 3) During interview, management reported the record for Child #1 did not have documentation related to child updates and confirmation of up-to-date information in the child's record as required by 8VAC20-780-420 E 3.

Plan of Correction: The center will send letter to require updated. Parent conferences will be held over the next two weeks. Information will be kept on the child's record.

Standard #: 8VAC20-780-550-D
Description: Based on a review of documents and interview, the center did not implement a monthly practice evacuation drill.

Evidence: 1) The licensing inspector observed the emergency drill log for the year 2022. An evacuation drill was not documented for April or May 2022. 2) During interview, management reported the drills were not practiced during those months. The center shall implement a monthly practice evacuation drill.

Plan of Correction: Corrected. All required drills are being practiced and documented as required.
