Comments:
An announced complaint inspection was conducted on 11/14/2022 from 2:00 p.m. to 3:10 p.m. There were 39 children present, ranging in ages from 3 to 5 years old. The inspector reviewed compliance in the areas of administration, staffing and supervision, medication, special care and nutrition. Interviews were conducted in conjunction with the local Child Protective Services.

The information gathered during the investigation supports the allegations, so the complaint is determined to be valid.

Complaint related: Yes
Description: Based on interviews, the center failed to obtain written authorization for over the counter diaper cream noting any known adverse reactions.
Evidence:
1. Staff #1 confirmed that the center did not obtain the appropriate written authorization for use of over the counter diaper cream for Medications #1 and #2.
2. Staff #1, Staff #2 and Staff #3 confirmed that the diaper creams (Medication #1 and #2) were used.

Based on interviews, the center failed to keep a record of use of over the counter diaper cream.
Evidence:
1. Staff #1 confirmed that the center did not maintain records of use for over the counter diaper cream for Medications #1 and #2.
2. Staff #1, Staff #2 and Staff #3 confirmed that the diaper creams (Medications #1 and #2) were was used and a record was not kept.

Plan of Correction: Program now has the VDSS authorization form for us of nonprescription over the counter skin
products. Staff has been trained to utilize the MAT medication dosage form each time they
apply the product to the child.
We have also instituted a diapering log for all children who are in pullups or diapers. Staff are
logging all information at each change. This log is posted in each bathroom by the changing
table.
