Comments:

An unannounced renewal inspection was conducted on November 7, 2022 from approximately 11:15 am to 12:15 pm. There were 21 children in attendance and a total of 4 staff present. 5 staff records and 3 children?s records were reviewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed playing outside and working in small groups while the licensing inspector was on the premises. The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted. Violations were cited as a result of this inspection. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 11/17/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #:	8VAC20-770-60-B
Description:	Based on records review, and observation, the center failed to ensure that staff complete a sworn statement prior to employment. Evidence: The record for staff #2, date of employment October 2021, and seen working alone in classroom #2 with children, did not contain a sworn statement.
Plan of Correction:	Per the Director staff #2 "completed the sworn statement."
Standard #:	8VAC20-780-160-A
Description:	Based on records review, and interview, the center failed to ensure that staff obtain and submit the results of a negative tuberculosis screening on or before the first day of employment. Evidence: The record for staff #2, date of employment October 2021, did not contain documentation of a tuberculosis screening. When interviewed, the Director stated that the tuberculosis screening had not been completed for staff #2.
Plan of Correction:	Per the Director staff #2 "is a substitute teacher. She will schedule an updated TB test."
Standard #:	8VAC20-780-160-C
Description:	Based on records review, the center failed to ensure that staff obtain and submit the results of a follow up tuberculosis screening at least every two years from the date of the initial screening. Evidence: The record for staff #1, date of employment 10/19/2018, contained a tuberculosis screening dated 10/19/2018. 2) The record for staff #3, date of employment, June 2019 contained a tuberculosis screening dated 8/21/2019. 3) The record for staff #5, date of employment September 1994 contained a tuberculosis screening dated 12/19/2018.
Plan of Correction:	Per the Director staff #1 "is updated." Staff #3 "is updated." Staff #5 "has scheduled her appointment...will be updated by end of Dec 2022."
Standard #:	8VAC20-780-530-A-1
Description:	Based on records review, and observation, the center failed to ensure that at least one staff in each classroom or area where children are present have, within 90 days current certification in CPR. Evidence: The record for staff #2, date of employment October 2021, and seen working alone in classroom #2 with a group of children, did not contain documentation of CPR training.
Plan of Correction:	Per the Director staff #2 "is a substitute teacher. She will not be in the classroom without a CPR trained staff member moving forward. She will attend the upcoming training in 2023"
Standard #:	8VAC20-780-530-A-2
Description:	Based on records review, and observation, the center failed to ensure that at least one staff in each classroom or area where children are present have within 90 days current certification in first aid. Evidence: The record for staff #2, date of employment October 2021, and seen working alone in classroom #2 with a group of children, did not contain documentation of first aid training.
Plan of Correction:	Per the Director staff #2 "is a substitute teacher. She will not be in the classroom without a First Aid trained staff member moving forward. She will attend the upcoming training in 2023."

Description:	Based on records review, and interview, the center failed to document that an evacuation drill took place each month. Evidence: The last evacuation drill that was documented was on 9/23/22. When asked, the Director stated that they do not conduct monthly evacuation drills.
Plan of Correction:	Per the Director "moving forward, the JK program will conduct monthly evacuation drills."
