Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on October 19, 2022 regarding allegations related to staffing and supervison and special care provision and emergencies. The inspection was initiated on October 25, 2022. During the inspection, interviews with staff were completed, observations were made in the center, video footage was reviewed and documentation was reviewed. The preponderance of the evidence gathered during the investigation support the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Description: Based on staff interview and video footage, the facility did not ensure the staff are ensuring care protection and guidance.
Evidence:
1- On October 13, 2022, Staff #1 walks over to a child and kicks the basket. Child #1 is on the floor and the basket hits child #1. Child#1 moves his hand to his face. Staff#2 goes to child #1 approximately10- 15 seconds after the incident to comfort child #1.
2- Staff #1 stated she kicked the toy bucket when playing with another child and the bucket hit child #1 in face. Staff #2 stated Staff #1 kicked the bucket to move it and the bucket hit child #1 in the face.
Staff #1 and staff #2 confirmed they did not contact the person in charge to inform them of the incident. Staff #2 stated there was no mark on the face initially and no first aid was provided.
3- Staff #3 stated after nap child #1 woke up with mark on his face. Staff #3 saw a mark and felt a bump on his face. Staff #3 stated ice pack was provided to the child#1.
4- Staff #3 contacted staff #2 to find out what happened to child # 1 and she stated child#1 was hit by the basket kicked by staff #1. Staff #3 stated the inital report provided in the system was edited after it was sent to the parent.

Plan of Correction: We have revaluated out injury prevention plan and injury reports. The injury reports are given to teachers to complete. We have policies and proccedures for all incidents at anytime even if no marking or with marking.

There are many communication tools used when there are injuries. It will be sent in Procare, parents will be called and hard copy and parent will need to sign hardcopy. There will be three means of communuication .

We trained staff on appropriate wayof moving toys and you should never use feet for any means to move anything, being mindful of surrounding and taking all matters serious and appropriate action to do . There were trainings on November 3 and November 4th.

Description: Based on record review, the licensee did not ensure injury report contained all required information.

Evidence: The following was missing on the injury reported provided on October 25, 2022 , the date and time parent was notified, staff present, how parent was notified, any future action to prevent recurrence of injury, two staff signature or one staff signature and parent signature.

Plan of Correction: We updated injury the report with how parent was notified, staff present , include preventive action to follow, signatures from supervisor , staff and parent and included the time and dates and who was in location during the incident.
