Comments:
A self-report inspection was conducted jointly with local agency representatives and the inspector was initiated on August 19, 2022 and completed on October 25, 2022 regarding concerns of programs. Interviews with staff and observations of were made at the center, documentation and video footage were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-410-1
Description: Based on observation of center's video footage, interview, and review of documentation, the center did not ensure all staff refrained from forbidden actions.


Evidence: On August 19, 2022, there was a review of video footage capturing the incident on August 18, 2022 involving Staff#1 interactions with Child#1 and Child#2 in the classroom.
The observations consists of the following:
1. Child#1 and Child#2 were separated and placed on opposite sides of the green wall.
2. Staff#1 roughly grabbed Child#1 by the arm and directed child to sit down on the floor. The second time Staff#1 grabbed child by the arm while Child#1 was standing and roughly sat child back down on the floor . The third time Child#1 was sitting down and Staff#1 grabbed child by the arm and roughly sat the child down on the floor.
3. Child#2 attempted to go to the other side and Staff#2 immediately grabbed child by the hand, roughly and directed child to sit down.
4. Interview with staff confirmed observations in the video footage.
5. Documentation reviewed confirmed incident observed at the center.

Plan of Correction: Preventative measures include providing a written copy off all policies during employee orientation. The Senior Manager or Assistant Manager will review specifically policy 1:1 and 1:4 to include scenarios and role playing.

9/20/2022 - A staff meeting was conducted to review center policies regarding appropriate teacher/child interactions. Staff participated in engagement activities and discussions on appropriate responses to various interaction scenarios. Handouts were provided to reinforce appropriate interactions.

Annually - The Senior Manager or Assistant Manager will conduct in-services to review teacher/child interaction policies including scenarios and role playing.

Immediately and ongoing - Coaching sessions will be offered to staff needing additional support.
