Comments:

An unannounced renewal inspection was conducted 11/3/2022 from 9:10am through 11:05am. There were 26 children directly supervised by 5 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 4 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were inspected. There were reportedly no medications on site. Children were observed engaging in free play, participating in circle time, dancing, and playing outside. Positive interactions between children and staff were observed during this inspection. There was an adequate supply of age appropriate materials for the children. There was an adequate number of CPR/First Aid and Daily Health Observation trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Description:	Based on observation and staff interview, the center did not ensure that mattresses are covered with a waterproof material which can be cleaned and sanitized. Evidence: The crib mattress for Child #6 has tears on all four sides of the mattress exposing the foam filling and making it no longer waterproof or able to be cleaned and sanitized.
