Discussed stairs and hand rail.

Comments:
An announced SHSI was conducted from 1030-1145 after the Inspector received a move notice. The vendor was present withe 3 children for a total of 6 points and Inspector observed children playing while watching television. The Inspector noted the back yard could not be used by children due to unsafe equipment but the vendor stated she takes them to the park and has written permission to take them. The Vendor did not have a copy of CPS check for herself an a household member but a call to the local agency verified the checks were complete. The Vendor has completed the Health and Safety training and her First Aid and CPR expires in February, 2020. Please call with questions 703-479-4679

Description: Based on review of 3 children's files, it was determined the vendor did not maintain all required information.
Evidence:
1. 3 of 3 files maintained by the vendor did not contain acknowledgement of the sick policy.
2. Child #1 had a name and address for an emergency contact but it was only one name and not legible.

Plan of Correction: I will have the parent give me the first and last name of the emergency contact and I will have parents sign the sick policy.
