Comments:
An unannounced monitoring inspection was conducted 11/29/2022 from 9:00am to 11:00am. There were 35 children directly supervised by 8 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 5 children?s records, 3 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed building with legos and blocks, practicing handwriting, listening to Green Eggs and Ham storytime, and doing a craft learning about the color yellow. Infant care including feeding, tummy time, and diaper changes were observed. Positive interactions between children and staff were observed during this inspection. There was an adequate supply of age appropriate materials for the children. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, the center did not ensure that all staff submit documentation of a negative tuberculosis screening completed within the last 30 days at the time of employment.

Evidence: Staff #1 (Date of Hire: 8/15/2022) had a tuberculosis screening on file dated 6/22/2021. Staff #3 (Date of Hire: 9/19/2022) had a tuberculosis screening on file dated 6/25/2021.

Description: Based on review of medications, the center did not ensure that prescription medication is given to children only with written authorization from the parent.

Evidence: Child #6 had two emergency medications on site that were not accompanied by a parental authorization granting the center permission to administer the medications.
