Description: Based on a review of staff records, staff have not submitted documentation of a negative tuberculosis screening.
Staff #1, date of hire 7/17/2021 has not obtained a TB screening.

Plan of Correction: Staff will obtain their TB screening and provide the proper documentation for their record.

Standard #: 8VAC20-780-240-A
Description: Based on a review of staff records, staff have not obtained the training in The Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence: Staff #2, date of hire 8/1/2022 has not completed the 10 hour Preservice orientation training within 90 days of hire.

Plan of Correction: Staff will take the required 10 hour training and file certificate in staff's record.

Standard #: 8VAC20-780-240-B
Description: Based on a review of staff records, Staff have not completed orientation training in prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: Staff #2, date of hire 8/1/2022 does not have documentation that she has obtained the required orientation training.

Plan of Correction: Management will provide orientation training and document that training.

Standard #: 8VAC20-780-245-L
Description: Based on a review of staff records and interview, staff have not obtained within the last three years instruction in performing the daily health observation of children.
Evidence: There is no one currently on staff that has been trained in Daily Health Observation of children. Staff were last trained 8/11/2017.

Plan of Correction: Staff will take Daily Health Observation training.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides are not kept in a locked place using a safe locking method that prevents access by children.
Evidence: Disinfectants were observed in 6 classrooms on top of the counters and under the classroom sinks. These disinfectants were accessible to children and not locked in a safe location.

Plan of Correction: All disinfectants will be locked.
