A monitoring inspection was initiated on November 28, 2022 and concluded on November 29, 2022. There were twelve children present, ranging in ages from three to four years old with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records, two medications, and two staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-160-A Description: Based on review of two staff files, program failed to ensure that staff submit documentation of a negative tuberculosis (TB) screening within the last thirty calendar days of the date of employment for one staff.
Evidence:
The date of employment for Staff # 2 is10/31/2022. The negative TB screening in the file was dated 9/16/22.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-200-A Description: Based on review of two staff files, program failed to ensure that there was a qualified program director on site for at least fifty percent of the center's hours of operation.
Evidence:
1. Neither staff that were on site qualifies as a program director.
2. The administrator confirmed that the qualified staff member that was previously on site is at another location and administration was unsure whether would return. Staff #1 is currently working on completing CDA credentials, but with staffing issues, unable to meet requirement at this time. .Plan of Correction: Not available online.
